Sport Accident Insurance Information

Acute, sport-related injury while training or competing

Step 1: File the Sport Accident Claim with UMR

  • If you are enrolled in EAHI, your provider needs to file a claim with UMR as your primary insurance.
    • It is your responsibility to provide your provider with your UMR member ID card to process the claim correctly.
  • If you are not enrolled in EAHI, please wait for your personal insurance to proccess. Once completed, any remaining out-of-pocket expenses can be applied to your NGB Sport Accident Policy, as outlined in Step 2.

Step 2: File the Claim with Your NGB's Sport Accident Insurance

  • If you are enrolled in EAHI, UMR will re-process the claim after receiving your information and apply the cost to the $25,000 deductible.
    • It is your responsibility to reach out to your NGB and learn how to file a claim with their Sport Accident Insurance (see below on how to do this).
      • NOTE: Your NGB Sport Accident Insurance will become your secondary insurance. Your EAHI or private health insurance will remain your primary insurance.

​Step 3: Provide the Sport Accident Information to UMR

  • Once the claim with UMR is filed/after you have received treament. UMR will need additional information about the claim. This request will arrive in your UMR online account, highlighted in ​red. 

    • It is your responsibility to fill out this questionnaire (see options below)
      • Login to your UMR online account and complete the sport accident questionnaire.
      • Call 800.207.3172 to answer the questions via phone
  • NOTE: The claim will be listed as denied until you submit this information, and you will be responsible for payment until the information is submitted.
  • NOTE: You will need the denied Explanation of Benefits (EOBs) from UMR or your private insurance carrier to submit to secondary insurance.
  • NOTE: You may be responsible for deductibles, co-payments, or other out-of-pocket expenses associated with your NGB's Sport Accident Policy.

How to Activate USATF Sport Accident After Reading and Completing Steps 1 & 2

You will need to fill out the USATF Sport Accident Insurance Claim Form.

  • In the Where it happened section, please put the location you were at when the injury happened (ex. USATF Nationals, practice, etc.)
Once you have completed the form, please email or fax it to NAGHA Claim Services:
  • ​Email: claims@nahga.com
  • Fax: 207-647-4569

If You Receive a Medical Bill

Any medical bills you have received for this injury can also be sent directly to NAGHA Claim Services (email or fax). Specific directions of what type of bill to send can be found on the second page of the medical claim form.

Note, it may take 2-3 weeks for the claim form to be processed and the insurance activated after you send it in. To check on the status, you can call NAGHA Claim Services at 800-952-4320.

Do NOT ignore any bills that you might receive in the mail during the time your claim is being processed. Keep in communication with the provider to avoid any bills being sent to collections.
