Public Disclosure — How to Request and Receive USATF's Covered Tax Documents

Below are the steps to request and receive USATF’s public disclosure covered tax documents. As per IRS requirements, requests for covered tax documents can be made in-person at USATF’s place of business, or in writing (faxed, emailed/contact form, or mailed). The request must include:

  • Name of the organization that filed the requested return(s) (i.e., USATF)
  • Requestor’s name
  • Requestor’s mailing address
  • Name of tax document(s) being requested
  • Years of the document(s) being requested (up to past 3 years)
  • Requestor’s telephone number or email address

This request can be:

  • Made in person at:
    • USATF National Headquarters
      130 E Washington St, Suite 800
      Indianapolis, IN 46204

  • Or, mailed to:
    • USATF National Headquarters
      Attn: Finance Dept
      130 E Washington St, Suite 800
      Indianapolis, IN 46204

  • Or, submitted by completing the Contact Form on USATF’s website: 
    • https://www.usatf.org/contact-us
  • Or, faxed to:
    • 317-261-0514

Requests are not accepted by telephone or email other than the contact form mentioned above.
 
USATF will provide by mail, the covered requested tax documents within 30 days from the date the request was received. Examples of covered tax documents include IRS Form 990 and IRS Form 990-T. The requestor will be charged for actual postage, as allowed by the IRS, prior to mailing the document(s). Additionally, non-members of USATF will also be charged copy fees of $0.20/page. USATF will respond within 7 days from the date the request was received with the total fees owed. Payment is required prior to mailing. Instructions for payment will be made available within that communication. Acceptable forms of payment for written requests include certified check, money order, credit card or personal check (must clear the bank before the request is considered paid in full).
 
Requests made in-person at USATF’s place of business will be completed the same day. Paper copies of the requested documents will be provided. The copying fee of $0.20/page applies to non-members of USATF and is required to be paid prior to the requestor receiving the documents. Instructions for payment will be made available at the time of the request. Acceptable forms of payment for in-person requests are cash or money order.
 
Please note that other documents and financial statements, audited or otherwise, are not included in the public disclosure covered tax documents defined by the IRS and are not required to be provided except as posted on USATF’s website.
