Entry Information for U.S. Citizens
1. What an entry includes
- Meet entry form
(PDF - via meet website)
- Name and address
- Birth date
- Age division (based on your age on September 4, 2007)
- Events
- Fee calculation (in Euros)
- Signature and date
- Meet Companion Entry Form (if applicable; see "Meet entry
form" link above)
- USA supplemental entry form
(PDF)
- Fee calculation in U.S. dollars
- Uniform selection and size
- Uniform shipping address (no post office boxes)
- 2007 USATF membership number (available online
or via your local Association)
- Payment information (credit card, bank check, money order,
personal check) No refunds
- Proof of age (copy of birth certificate or passport only - no driver's licenses)
Entries must be received in Indianapolis
by May 14, 2007. No entries will be accepted after this date. Please allow
sufficient mailing time to meet this deadline.
2. Where to mail entries and other forms
DO NOT MAIL ENTRY FORMS TO ITALY!
All entries for U.S. citizens must be mailed to:

Housing Forms should be mailed directly to Firma Eventi (the meet's
official travel agency) at the address on the form.
