1. What an entry includes
- Meet entry form
(PDF - via meet website)
- Name and address
- Birth date, written with the day first (example: March 20, 1955 is written
"20 03 1955")
- Age division
- Events
- Fees (in Euros) on the reverse*
- Signature and date
- USA supplemental entry form
(PDF)
- Fee calculation in U.S. dollars*
- Uniform selection and size
- Uniform shipping address (no post office boxes)
- 2005 USATF membership number (available online
or via your local Association)
- Payment information (credit card, bank check, money order,
personal check)
- No refunds
- Proof of age (copy of birth certificate or passport only - no driver's licenses)
* To avoid meet late charges, entries must be received in Indianapolis
by May 6, 2005. If entries are received after May 6, but on or before May
27, a $68.00 late fee must be added to your entry fees. From May 28 to June 17,
add $135.00. NO entries will be accepted after June 17, 2005. Please allow
sufficient mailing time to meet these deadlines.
2. Where to mail entries and other forms
DO NOT MAIL ENTRY FORMS TO SPAIN!

Housing and Medical Forms should be mailed directly to Spain (the
addresses are at the bottom of the forms):
3. Confirmation
You will receive a postcard from USATF when it receives your entry and a
separate confirmation from Spain during the summer. Check your confirmation from
Spain and make sure that all the information is correct. If there are any
errors, contact Philip Greenwald (greenwaldp@att.net
or 212-595-2486 before 9:00 p.m. ET) immediately.
