Entry Information
You must be a 2005 USATF member to enter the Championships.
If you do not have a 2005 membership, please complete a membership
application first. You must also be a U.S. citizen and meet the eligibility
requirements and qualifying
standards.

The entry fee for the Championships is $10 per athlete. Late entries incur an additional
$25 late fee per athlete. All fees are non-transferable
and non-refundable.
Entry Forms
Online entries must be paid for by credit card ("VISA, the only card
accepted by USA Track & Field").
Other Required Documents
- Verification of Age (a copy of your passport or birth certificate)
Athletes will not be considered to be entered into the meet until ALL
required documents have been received. Athletes missing required documents will
be listed as "pending" on the Status of Entries page of the USATF
website until all such documents are received. Athletes still missing required
documents at the end of the declarations period will automatically be scratched
from the competition.
Entry Process
- Online entry is the only method of entry. No other form of entry will be
accepted.
- A 2005 USATF membership is required to enter (please join
online if you are not a member).
- To enter online, you will be required to provide your 2005 USATF
membership number and password (click here to retrieve
your password if you don't know it).
- A Help Line is available for those who need help with the entry process.
For assistance call 317-261-0478 x351 during normal business hours, 8:30
a.m. - 5:00 p.m. CT, Monday through Friday.
- For entries close to the deadlines: although we will make
every reasonable effort, we cannot guarantee responses to requests for passwords.
- After completing your entry online, please fax a copy of the
information page of your passport or birth certificate to the USATF National
Office at 317-261-0481.

Declaration Process
Once properly entered, all athletes must complete the declaration process.
This process confirms your intention to compete in the Championships and allows
you to update your qualifying mark.
- Online is the only method of declaration. No other form of declaration
will be accepted.
- Each athlete or his representative must declare whether that athlete will
or will not compete. The person declaring may submit any corrected or
improved mark.
- Athletes not declaring by the declaration deadline will be scratched.
- To declare online, you will be required to provide your 2005 USATF
membership number and password (click here to retrieve
your password if you don't know it).

- Petitions for Late Declarations
- Petitions may be filed by calling 317-261-0478 x351.
- Athletes submitting a petition for late declaration will be assessed
a $25 late fee payable by credit card only.
- Decisions on late declaration petitions will be acted upon by a
special committee no later than Wednesday, June 22, 2005.
- An athlete whose petition is successful will be notified as soon as
the decision is final.
