Entry Information

You must be a 2004 USATF member to enter the Championships.  If you do not have a 2004 membership, please complete a membership application first.  You must also meet the eligibility requirements.

UPDATE:  Entry deadline extended!  Entries will be accepted online through 6/26/04 at 3:00 a.m. ET.

The entry fee for the Championships is $20 per athlete (regardless of the number of events entered).  Late entries incur an additional $30 late fee per athlete.  All fees are non-transferable and non-refundable.

Clubs that have 20 or more athletes who compete at the event will receive a $5-per-athlete rebate. Please allow 8-10 weeks for processing.

Entry Forms

Online entries must be paid for by credit card (USATF prefers VISA; MasterCard and Discover also accepted).

Entry Process

  • The only method of entry is online.
  • A 2004 USATF membership is required to enter (please join online if you are not a member).
  • To enter the Championships, you will be required to provide your 2004 USATF membership number and password (click here to retrieve your password if you don't know it).
  • For entries close to the deadlines:  although we will make every reasonable effort, we cannot guarantee responses to requests for passwords after 12:00 noon ET on those days.

Declaration Information

Once properly entered, all athletes must complete the declaration process and declare whether he/she will or will not compete in each event he/she has entered.

Notes

  • Declarations will be conducted online.
  • In order to make a declaration, please have the athlete's:
    • 2004 USATF membership number
    • Membership password (click here to retrieve your password if you don't know it)
    • Name
    • Affiliation
    • Event(s) declaring
    • Event(s) scratching
    • Qualifying event and qualifying mark
    • Name, location, and date of event where qualified; meet director and facility names
    • Phone number for call-back questions
  • Athletes not declaring by 5:00 p.m. ET, Wednesday, June 30, 2004 will be scratched from the competition.
