Entry Information
You must be a 2004 USATF member to enter the Championships.
If you do not have a 2004 membership, please complete a membership
application first. You must also be a U.S. citizen and meet the eligibility
requirements and qualifying
standards.

The entry fee for the Championships is $10
per athlete (regardless of the number of events entered). Late entries incur an additional
$25 late fee per athlete. All fees are non-transferable
and non-refundable.

Online entries must be paid for by credit card (USATF prefers VISA).
Other Required Documents
- Verification of Age (a copy of your passport or birth certificate)
Athletes will not be considered to be entered into the meet until ALL
required documents have been received. Athletes missing required documents will
be listed as "pending" on the Status of Entries page of the USATF
website until all such documents are received. Athletes still missing required
documents at the end of the declarations period will automatically be scratched
from the competition.
Entry Process
- Online entry is the only method of entry. No other form of entry will be
accepted.

- A 2004 USATF membership is required to enter (please join
online if you are not a member).
- To enter the Championships, you will be required to provide your 2004 USATF
membership number and password (click here to retrieve
your password if you don't know it).
- For entries close to the deadlines: although we will make
every reasonable effort, we cannot guarantee responses to requests for passwords.
- A Help Line is available for those athletes who need assistance with the
entry process. For assistance, call the USATF National Office at
317-261-0478 x351 during normal business hours, 8:30 a.m. - 5:00 p.m. CT
Monday-Friday.
- After completing your entry online, please fax a copy of the
information page of your passport or birth certificate to the USATF National
Office at 317-261-0481.

Declaration Process
Once properly entered, all athletes must complete the declaration process.
This process confirms your intention to compete in the Championships and allows
you to update your qualifying mark.
- Online is the only method of declaration. No other form of declaration

- Each athlete or his/her representative must declare whether that athlete
will or will not compete in each event entered. The person declaring may
submit a corrected or improved mark.
- Athletes not declaring by the declaration deadline will be
SCRATCHED.
- To declare, you will be required to provide your 2004 USATF

your password if you don't know it).
- For declarations close to the deadlines: although we will make

Declaration Petition Process
- An petition process will be
available for those athletes who wish to declare after the close of
declarations.
- An petition may be made by calling 317-261-0478 x351 during the petition
period.
- Petitions must be accompanied by a non-refundable $25 petition fee.
- The petition fee must be paid by credit card. USATF prefers VISA.
- Decisions on late declaration petitions will be acted upon by a special
committee no later than 5:00 p.m. CT on Tuesday, June 22, 2004.
