Entry Information
You must be a 2002 USATF member to enter the Championships.
If you do not have a 2002 membership number, please complete a membership
application first. You must also be a U.S. citizen and meet the eligibility
requirements and qualifying
standards.
The entry fee for the Championships is a one-time fee of $25
per person (regardless of the number of events entered). Late entries incur an additional
$75 late fee per person. All fees are non-transferable
and non-refundable.

- The entry deadline for on-time online entries is June
11, 2002 at 5:00 pm ET. Late entries will be accepted online
until June 13, 2002 at 11:59 pm ET.
- To enter the Championships, you will be required to provide your 2002 USATF
membership number and password (directions for receiving your password, if
you don't know it, are provided on the entry form--see below).
- If you already have your 2002 USATF number, you may proceed
to the entry form.
- If you do not yet have a 2002 USATF number, you'll need to complete a membership
application online first (you will receive your membership number
instantly). Then return to this page and go to the entry
form.
- For entries close to the June 11th and 13th deadlines: although we will make
every reasonable effort, we cannot guarantee responses to requests for
member passwords after 3:00 pm ET on those days.

- To enter the Championships, you will be required to provide your 2002 USATF
membership number. If you are not already a member, please join

- By postal mail - All mailed entries must be received by
USATF by 5:00 pm ET
on Tuesday, June 11 (or received by 11:59 pm ET on Thursday, June 13 for late entries). Make

- By fax - All faxed entries must be received by USATF by 5:00 pm ET
on Tuesday, June 11 (or received by 11:59 pm ET on Sunday, June 13 for late entries).
Faxed entries must be paid for by credit card. Fax the completed application and payment

Declaration Process
Once properly entered, you must complete the declaration process:
- Each athlete must declare whether he/she will or will not compete in each event
he/she has entered.
Declarations must be done by telephone 317-261-2629 on Friday, June
14, from 10:00 am - 6:00 pm
ET. Fax and mail
declarations will NOT be accepted.
- When making a declaration, please have all the following information
available:
- Athletes name
- Name of declarer
- Event declaring
- Event scratching
- Best qualifying mark
- When and where mark was made, including meet name (facility, city,
state), phone number (home and work, if available), and name of meet
director.
- Phone numbers (day and night) and e-mail addresses of athlete and
his/her coach/representative.
- At the time of declaration, you may update your qualifying performance.
The burden of accuracy for all declarations will be on the athlete or
his/her coach/representative. All entry marks will be checked and
non-verifiable marks will not be honored.
- Athletes who do not declare by the deadline date and time will be
scratched.
- Petitions for late declaration (317-261-2629) will be accepted until 1:00 pm ET on
Monday, June 17, 2002 and must be accompanied by a non-refundable $100 late
declaration fee.
