  • Complete top section - personal information, 2008 USATF #, DOB, etc
  • $10 PENALTY FEE If 2008 USATF # IS NOT ON ENTRY FORM
  • Complete Section 1 - List your choice of events and include best times/marks
  • Times/Marks must be obtained in a USATF sanction meet, high school meet or other formal competition
  • Leave Sections 2 & 3 blank at this time
  • Note: No changes or additions to events will be allowed after Tuesday June 17
  • Athlete AND Parent MUST SIGN form at bottom
  • Attach proper fee -CASH OR MONEY ORDER ONLY - Payable to USATF Three Rivers
  • INCLUDE LATE FEE IF POSTMARKED AFTER JUNE 9

    • Confirmation will be required if a problem arises with your entry

ELIGIBILITY & AGE GROUPS
Individuals:  Only US citizens, aliens living in the United States, and foreign exchange students are eligible to compete in these Championships.  See the USATF Rules for more information and exceptions.  A competitor must compete in his/her age division only.  Competitors in the Bantam and Midget divisions may compete in a maximum of three events, including relays.  Competitors in the Youth, Intermediate & Young Men/Women divisions may compete in a maximum of four events, including relays.  Combined events are not included in the event count.  All athletes must be 2008 members of USATF in good standing
Relay Teams:  Only registered 2008 USATF member clubs may enter a relay team(s).  All athletes representing the club must be affiliated with and members of that club as part of their USATF membership.
PROOF OF AGE REQUIRED: DOB Verified 2008 USATF Membership Card or a copy of a B/C

*This age group advances to the Region 2 Championship but not the Nat'l JO Meet. Athletes born 1998 & later wishing to compete in that meet must compete as a Bantam
**Athletes born in 1989 are also eligible if they do not turn 19 on or before 7/29/2008
