2002 USATF Annual Meeting - Registration
- The early-bird rate fee of $150.00 is in effect only for those
registrations postmarked on or before November 12th. After
November 12th, the cost will be $200.00. NO EXCEPTIONS!
- VERY IMPORTANT! Please read our Refund/Cancellations policy. Refunds can
be issued only to those registrants who notify the National Headquarters of
their cancellation prior to November 12, 2002, less a $25.00 processing fee.
Cancellations received by November 22nd will receive a 50% refund, less a
$25.00 processing fee. Cancellations received after November 22nd will
not be refunded.
- Please indicate your name and affiliation EXACTLY as you want it to appear
on your name badge. There will be no reprinting of name badges during the
meetings, so please PRINT CLEARLY, as there will be a $10 replacement fee
for lost or misprinted name badges.
- Please indicate your 2002 USATF membership number, in the space provided,
and make sure that your registration form is completed properly. Our Bylaws
require that all participants of the Annual Meeting be members of USATF. If
you were not a member in 2002, you may obtain a number for 2003 by
completing the registration form and including an additional $20.00 with
your registration fee. If you are not able to provide proof of membership at
the time of the meeting, you will be required to purchase a 2003 USATF
membership number onsite. Please complete ALL required fields if you are
purchasing a 2003 membership.
- In order to support our various grass roots programs and initiatives,
USATF will be conducting a 50/50 raffle at this year's Annual Meeting - open
only to registered delegates. Each entry is $25 and proceeds will be evenly
distributed between the winning entrant and USATF. The organization's
portion will be used to support our grass roots and developmental programs.
